Community Matrons – Warrington


General introduction

Community Matrons work with patients to support them in managing their own health and to help prevent avoidable hospital admissions, as well as to facilitate early supported discharge when appropriate.

This is achieved through advanced clinical practice, case‑management skills and close engagement with other health and social care professionals.

Community Matrons have advanced training that enables them to clinically examine, diagnose and prescribe for patients when required, allowing early treatment and effective management of symptoms.

How to access this service

This service is available to people aged 18 and over who have multiple long‑term conditions or a range of complex health needs. Patients must be registered with a Warrington GP.

People accessing this service are often at risk of hospital admission or are frequent users of GP or out‑of‑hours services.

Individuals can self‑refer, or a referral can be made by a health professional.
