General introduction

We provide comprehensive physical and mental health reviews delivered by registered nurses across Halton.

Who we are

We are a team of experienced mental health and general nurses based at The Bridges Learning Centre, Widnes.

Working closely with all GP practices in Widnes and Runcorn, we offer annual physical health checks tailored to your individual needs.

Our services

  • annual health checks – blood pressure, BMI and an overall health review
  • lifestyle advice – nutrition, exercise and wellbeing guidance
  • referrals – support to access smoking cessation, alcohol/drug services and specialist teams
  • blood tests – cholesterol, diabetes and other key health markers
  • additional tests – tailored checks based on your needs
  • medication support – discussions about your medicines and answers to any questions
How to access this service

You can access our service if you are aged 18 or over, are registered with a GP practice in Widnes or Runcorn, and are currently on the severe mental illness (SMI) register.

Referrals are made directly by your GP. If you are eligible, your GP will send your details to us.

Once we receive your referral, a member of our team will contact you to arrange an appointment.

Appointment information

We will phone or write to you to arrange a clinic appointment or home visit. Please tell us your preferred method of contact.

If you need to cancel or change your appointment, contact us or contact your GP practice as soon as possible. We will help you rebook at a convenient time.

If attending the clinic is not suitable, we can arrange a home visit. Please mention this when we contact you.
