Palliative Care (Community Specialist Team) – Halton


General introduction

The community specialist palliative care team is a community‑based service made up of clinical nurse specialists with the expertise to provide specialist palliative advice and support for people with life‑limiting illnesses.

This is a specialist advice service offering support with:

  • complex pain and symptom management
  • psychological and emotional support
  • signposting to other appropriate services

The clinical nurse specialists work closely with patients, families, carers and a wide range of professionals as part of a multi‑disciplinary team, including:

  • integrated community nursing teams
  • general practitioners
  • acute oncology teams
  • consultants
  • outpatient and day care departments
  • family support services
  • other clinical nurse specialists
  • pharmacists
  • social services and voluntary sector services
  • physiotherapists and occupational therapists

The service is offered in the patient’s own home or within our clinic at the Independent Living Centre in Runcorn.

How to access this service

The service is available to adults aged 18 or over who are registered with a Halton GP and have a palliative diagnosis.

Referrals can be made using our referral form by any health or social care professional working with people who have life‑limiting illnesses.

Self‑referrals can also be made by patients, families or carers by telephoning the office.
