Palliative Care Clinical Macmillan Nurse Specialists – Warrington


General introduction

The Macmillan Palliative Care Team is a community‑based team of clinical nurse specialists with the knowledge and skills to provide specialist advice and support to patients and their families living with a life‑limiting illness.

This includes people with conditions other than cancer.

This specialist service offers support with complex pain and symptom management, psychological and emotional wellbeing, and provides advice to GPs and district nurses. The team also signposts patients and families to other services when appropriate.

The clinical nurse specialists work closely with patients, their families and carers, as well as with other healthcare professionals within the multidisciplinary team. They also collaborate with specialist palliative care providers in the hospice and hospital.

The team is supported by a consultant in palliative medicine.

The service is available seven days a week from 8.30am to 4.30pm.

Please note: at weekends and on bank holidays there is a reduced service, with only one clinical nurse specialist available.

How to access this service

The service is for adults aged 18 and over who have a life‑limiting illness and who agree to being contacted by the specialist team.The service is offered in the patient's own home.

Referrals

Referrals can be made by any health or social care professional working with people who have life‑limiting illnesses.

The team also accepts self‑referrals from patients or their families. In these cases, further information will be requested from the patient’s GP.
