Intermediate Care – Warrington
General introduction
Intermediate care is a range of needs‑led, transitional and integrated services designed to improve patient outcomes by:
- promoting faster recovery from illness
- preventing unnecessary attendance or admission to an acute hospital
- preventing premature or avoidable admission to long‑term residential care
- maximising independence and reducing the need for long‑term care support
These services are delivered in partnership between community health care and the local authority and include:
- therapy-led rehabilitation
- personal care and social support
- nursing support
- medical support
The service works with people in a range of settings, including their own home or a designated care environment.
Intermediate care includes a programme of active rehabilitation, using an appropriate combination of occupational therapy, physiotherapy and medical or nursing input. The focus is always on supporting people to regain as much independence as possible.
Wherever possible, the service aims to help people stay at home or return home.
The therapy service operates Monday to Friday.
