Falls and Rehabilitation Service – Warrington


General introduction

The Falls and Rehabilitation Team is a group of health professionals who work together to assess all the factors that may increase someone’s risk of falling.

This service is the main point of referral for people who have experienced a fall.

A trained professional will carry out a full assessment, either in a clinic or at home if you are unable to attend in person.

The team will:

  • provide support and advice to other health professionals
  • ensure the falls pathway is followed correctly and that people are referred to hospital when needed
  • carry out detailed assessments to understand an individual’s risk of falling or developing osteoporosis
  • work to prevent falls across Warrington
  • work closely with the falls consultant, Emergency Department (A&E) and hospital teams
  • provide education for staff on falls prevention, osteoporosis and the use of first‑line assessment tools
How to access this service

Adult over 18 who live in Warrington and are registered with a Warrington GP can access this service.

Referrals are accepted for people who:

  • have experienced a fall
  • are at risk of falling
  • require a falls or osteoporosis assessment

Referrals will be accepted from GPs, consultants, health care professionals, social care professionals and nursing and residential care homes via the Enhanced Care Home Support Team.

This is a community‑based service. The team visits patients in their own homes or in residential, nursing or care homes.
