Enhanced Care Home Support Service – Warrington
General introduction
The Enhanced Care Home Support Service provides dedicated support, advice and education to all nursing and residential care homes within the Warrington area, helping to ensure high‑quality, equitable and appropriate care.
A triage service is provided through the Care Home Helpline, ensuring each patient is assessed by the most appropriate member of the team or by their own GP practice when required.
The team carries out comprehensive health needs assessments using a skilled mix of nurses, and works closely with GPs and staff within the Primary Care Networks (PCNs).
The service is delivered in partnership with primary care, secondary care, the local mental health trust, North West Ambulance Service, local provider services and local government services.
The service aims to provide:
- proactive and reactive assessments, delivered face to face or over the telephone
- end‑of‑life care, including advanced care planning and preferred priorities of care
- early supported discharge from hospital
- social and personal support
- education relating to end‑of‑life care and locally developed pathways
Tthe service also:
- follows the Falls Integrated Pathway
- reduces inappropriate GP visits, helping to reduce A&E attendances and emergency admissions
- undertakes monthly multi‑disciplinary team (MDT) meetings within care homes, involving the designated GP, care home staff and other relevant health professionals
