Across Warrington we have been working hard to bring our maternity and 0-19 services closer together as we progress with the integration of our clinical services.
By working more closely we are able to offer a service that enhances care for both children and families, at every stage of the maternity journey.
This joint approach is focused on improving continuity of care during the antenatal and postnatal periods, ensuring a more seamless journey for patients as they move from hospital care to being supported at home and by our community teams.
An example of how this is working is at the Living Well Hub, Warrington.
Our midwifery and 0-19 services work together to offer complementary support for parents, from maternity appointments to baby weighing and feeding support.
It means teams can identify health issues earlier, helping to reduce health inequalities, offer targeted support sooner and deliver better outcomes for children and parents alike.
